Benefit statements are mailed out once a year to the most recent address we have on file for members.  The information included on the statement is through the latest year ended December 31st.  If you did not receive your benefit statement, please complete the below form and we will get back to you as soon as possible.  We may need to verify additional information prior to sending the statement, but submission of the form will get the process started.
